Case Management
Case management, by definition, is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs. Through effective communication and the utilization of available resources, we promote quality, cost-effective outcomes.

A Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized concise plan for current and future needs with associated costs, for individuals who have experienced catastrophic injury or have chronic health care needs.
Motor Vehicle Accidents
Case management following a motor vehicle accident focuses on helping clients and their families navigate the often complex and overwhelming rehabilitation system. We coordinate medical, rehabilitation, and community resources to ensure timely access to occupational therapy, physiotherapy, psychology, and other allied health services. We act as the central point of contact between clients, treatment providers, insurers, and legal representatives, reducing stress and confusion while advocating for appropriate care and funding. Our goal is to support recovery, maximize independence, and help clients and their families feel confident that no detail has been overlooked.
Care Coordination
Care coordination brings clarity and structure to what can often feel like a confusing and fragmented system. We work closely with clients and families to align medical, rehabilitation, and community services into a seamless plan of care. Whether the need is supporting an older adult at home, guiding a family through the transition into a long-term care residence, or coordinating services for someone living with a complex injury or medical condition, we provide ongoing oversight and advocacy.

Discharge Planning
Transitioning from hospital to home or another care setting can be challenging without coordinated support. Our discharge planning case management ensures that clients leave hospital with the right services, equipment, and resources in place for a safe and smooth recovery. We collaborate with the healthcare team, community providers, and family members to arrange follow-up therapy, nursing support, personal care, and transportation as needed. By addressing both immediate needs and long-term considerations, we reduce the risk of complications or re-hospitalization and provide families with peace of mind during a critical time of change.
We support clients through every stage of their health and recovery journey. Our team assists with hospital discharge and transition planning, ensuring a safe return home or move into long-term care. We coordinate rehabilitation by matching you with the right professionals, including occupational therapists, physiotherapists, speech-language pathologists, social workers, neuro-physiotherapists, and more. We also arrange and attend medical and specialist appointments to ensure follow-up care is clear and seamless. For daily needs, we connect families with trusted personal support workers and nurses to provide consistent, compassionate care.
Health and Rehabilitation

Our team supports accessible housing and home modifications, including renovations, equipment, and assistive devices tailored to your needs. We also provide guidance on choosing and setting up mobility aids, communication tools, and adaptive technologies. Home and Daily Living

Family/Caregiver Support

Our team works with insurers, adjusters, and legal professionals for both motor vehicle accident (MVA) and private cases. We also connect clients with trusted legal support for wills, powers of attorney, guardianship matters, insurance claims, and more.
