Tri-Star Health Management Group is a dedicated, community-based organization specializing in personalized case management and support for individuals with catastrophic impairments, including but not limited to Traumatic Brain Injury, Spinal Cord Injury, Chronic Pain, Psychiatric Disorders, and Amputations, serving both children and adults. Our case managers also support individuals with complex medical conditions.

Case Management

Case management, by definition, is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs. Through effective communication and the utilization of available resources, we promote quality, cost-effective outcomes.

Our team provides detailed case management and consultation summary reports, providing a comprehensive overview of the client's progress and care plan. This documentation further ensures consistent communication with all parties involved in the care process. We provide thorough correspondence with clients, medical teams, and insurers to keep everyone informed and aligned.

Life Care Planning

A Life Care Plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis and research, which provides an organized concise plan for current and future needs with associated costs, for individuals who have experienced catastrophic injury or have chronic health care needs.

Our Approach

At Tri-Star Health Management Group our approach is centered around providing objective, unbiased, and cost-effective Life Care Plans/Future Cost of Care Reports that can be utilized as part of litigation, but also as a roadmap for an individual as they age with their injuries or medical condition.  

Every Life Care Plan undergoes a meticulous peer review process led by a senior life care planner, ensuring the highest standards of quality and reliability.

Standards of Practice

In alignment with the International Academy of Life Care Planners: Standards of Practice for Life Care Planners 4th Edition, our Life Care Plans aim to: ​

  1. Assist the evaluee in achieving optimal outcomes by developing an appropriate plan of rehabilitation, habilitation, prevention, and reduction of complications. This includes recommendations that contribute to the evaluee’s health, wellness, and quality of life.

  2. Provide health education to the evaluee and relevant parties, when appropriate.

  3. Specify services needed by the evaluee and the associated costs.

  4. Evaluate alternatives for care that take into consideration appropriate and least restrictive options for he evaluee. To communicate the objectives of the life care plan to the evaluee and relevant parties, as appropriate.

Life Care Planning: Offered Services

  • Preliminary Future Cost of Care Report

    • Typically, telephone or video interview; in-person meeting if deemed required.

    • Review of core medicals / reports only.

    • Research literature review as required.

    • Phone consultation with key therapists / specialists only if required.

    • Background & pre-incident functional status.

    • Incident summary.

    • Overview of impairments, current symptoms and functional status.

    • Overview of medical / rehab intervention.

    • Preliminary analysis of future care needs and cost analysis over a period of time or lifetime.

    • Completed by an Occupational Therapist and Case Manager/CCLCP.

    Comprehensive Future Cost of Care Report

    • In-person meeting with evaluee.

    • Document review.

    • Research literature review.

    • Phone consultation with relevant therapists / specialists.

    • Background & Pre-incident Functional Status; Past Medical History.

    • Incident Summary.

    • History of Medical Treatment & Rehabilitation.

    • Post-Incident Functional Status.

    • Future care needs and cost analysis outlining the following over a lifetime.

    • Form 53 included.

    • Completed by an Occupational Therapist or Case Manager/CCLCP.

    • Typically based on a file review and telephone interview with evaluee.

    • Overview of medical and rehabilitation background; current status.

    • Analysis of future care needs and cost analysis over a period of time or lifetime.

    • Completed by an Occupational Therapist and Case Manager/CCLCP.

    • In-person meeting with the evaluee.

    • Identify costs of services such as personal care, caregiving, housekeeping and home maintenance for which assistance is now required.

    • Identify current therapy needs at SABS and / or Market rates.

    • Completed by an Occupational Therapist or Case Manager.

    • Identify costs of lost services such as personal care, care-giving, housekeeping and home maintenance, for a deceased individual.

    • Completed by an Occupational Therapist or Case Manager.
