Heather Condello
Ms. Condello is a registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario and a member of the Canadian Association of Occupational Therapists. She has completed a Master of Clinical Science degree in Occupational Therapy and a Bachelor’s degree in Kinesiology.
Ms. Condello has over twenty years’ experience as an Occupational Therapist having spent the majority of her career as a community-based therapist working within the auto insurance sector; however, also spending time practicing in the public sector in outpatient burns, amputations and complex orthopedics and working with neurologically impaired clients including the prescription of assistive technology for such clients. As a community based Occupational Therapist, Ms. Condello’s practice has included in home assessment (including assessment of attendant care needs, home safety assessments, and comprehensive physical, cognitive and psychosocial assessments), discharge planning assessments, case management and occupational therapy treatment for clients with brain injury, spinal cord injury, burns, amputations, chronic pain and orthopedic injuries. She spent fourteen years managing the clinical practice at Complex Injury Rehab which included providing mentorship on clinical practice and reviewing thousands of occupational therapy assessment and progress reports.
She is currently a consultant Canadian Certified Life Care Planner with Tri-Star Health Management Group Inc. with a practice focus on completing life care plan reports. Ms. Condello also provides mentorship support to the other members of Tri-Star Health Management Group’s life care planning team. Her work as a mentor to other life care planners has included review of over two hundred future cost of care reports.
